Advanced Technology and Manufacturing Institute (ATAMI)

All people entering the facility must affirm and acknowledge that the following:

• I have read the OSU Quarantine and Isolation guidelines and do NOT meet the quarantine or isolation requirements per the date of my visit to the ATAMI facility.   
• I will wear a well-fitting mask as specified in those guidelines.

Notes: 

• Per CDC guidelines, a cloth or surgical mask is not considered tight fitting.  An N95 is considered tight fitting and is recommended.  ATAMI can supply N95 masks as needed.
• Refer to CDC guidelines to determine vaccination status.

• I am not ill. I do not have fever, chills, repeated shaking with chills, muscle pain, cough, shortness of breath, sore throat, new loss of sense of smell or taste, or other symptoms common to COVID-19 infection, flu, or colds.
